Description:
Spurwink Services is looking for an organized, family-focused and advocacy-oriented individual to join our Targeted Case Management team in our Westbrook office. We are seeking individuals who are highly organized, have good time-management skills, appreciation for ongoing professional learning, and computer competency. Case managers are required to have excellent communication/engagement skills with families and colleagues.
Duties:
Provide intake/assessment when clients are referred; Facilitate Individual Service Planteam meetings and develop a plan of care (ISP) for the child with input from the team. Case Manager is responsible in overseeing implementation of ISP, monitoring and documenting progress. Case Managers maintain regular contact with team membersand other community professionals involved with child; Ensure that clients and families are aware of all communityservices and resources. Assist families in accessing resources; and provide family support through phone contact andface-to-face meetings through home visits and community outreach.
Qualifications:
Bachelor's degree from an accredited four (4) year institution of higher learning with a specialization in social services or closely related field. Experience in social services preferred.
