6 Full-time 3 hours ago Full Job Description Position Summary We have an exciting opportunity for an experienced Health Coach (HC) to join our growing Lend a Hand program! Lend a Hand is a complex care management program embedded within the primary care setting.
As part of a multi-disciplinary care coordination team (RN and Social Worker), the Health Coach will work with patients throughout their participation in the program.
Beginning with the invitation outreach and initial follow-up telephone call, the Health Coach will work to introduce the program to eligible patients.
Then, throughout the continuum of Lend a Hand enrollment to discharge, the HC works closely with other Lend a Hand team members to create, monitor, and support patient-identified goals.
As a result, the relationship between the HC and the patient continues to develop.
It is unique in that the HC also represents a kind of peer support that patients often benefit from when working to improve health outcomes and navigate the health care system.

Required Minimum Knowledge, Skills, and Abilities (KSAs) Education:
High School Diploma preferred.
Bachelor's Degree preferred.
License/Certifications:
Valid Maine Driver's License.
Health Coach certification preferred.
Experience:
2 years' working in a social service or health care setting or equivalent educational setting preferred.
Demonstrated ability to work with individuals and families who have complex social and clinical needs preferred.
Bilingual preferred.
Sensitivity to cultural and ethnic diversity preferred.
Local community resident with good knowledge of the resources available in the community preferred.
Excellent organizational and critical thinking skills and ability to prioritize workload and to demonstrate flexible problem solving approach.
Excellent communication and interpersonal skills and abilities to respond to diverse needs from patients, families and employees.
Familiarity with medical terminology preferred.
Ability to project a pleasant and professional image even under stressful situations.
Ability to maintain confidentiality.
Must complete most duties with minimum direct supervision and demonstrate ability to collaborate with teams of individuals in diverse settings.
Ability to travel frequently to PCP practices and other sites where patients receive care.
Proficiency in computer use, internet and health information technology; knowledge of EPIC electronic medical record system preferred.

Estimated Salary: $20 to $28 per hour based on qualifications.
